Social Worker

Full time position with benefits.  South Lyon Medical Center needs a Nevada Licensed Social Worker to provide assessments, referrals, handle admissions, and discharge planning for both Long Term Care and Acute patients.  Candidate must have knowledge of Federal and State guidelines. As the Licensed Social Worker, you will work directly with our residents, patients and their families.  This position requires patience, compassion and communication.  It is our focus to make the transition for our residents and patients into our facility as smoothly as possible. 

The Social Worker will be responsible for assisting in the planning, developing, organizing, implementing, evaluating and directing the Social Services department at our facility in accordance with current existing Federal, State and local standards as well as our established policies and procedures to ensure that the medically related emotional and social needs of residents and/or patients are met and maintained on an individual basis.  The Social worker will also assist with residents and family members concerns and questions, make patient assessments, follow care plans, and perform discharge procedures.  Candidates must have knowledge and/or previous experience with the geriatric population and State regulations as they apply to skilled nursing and Long Term Care.  Eligible candidates must be able to pass a drug screening, physical exam and criminal background check.
